Self-Enrollment for Health Records Online

Self enrollment is available for patients who are age 18 or older that don't currently have access to Health Records Online, an online portal where you can view select medical information. Complete the form below to start the self enrollment process.

If you manage the health of a patient, talk to the patient's health care provider at their next visit to receive a personal invitation to Health Records Online. Self enrollment is not currently available for caregivers and guardians.

Medical Record Number Enter your Medical Record Number (MRN) that is found on your hospital wristband or on documentation provided to you at discharge. Enter only the numbers in the MRN, without leading zeros. For example, if your MRN is STH0012345, enter 12345.
